Essentials of Drug Review for Safer Patient Management


Medication errors including inappropriate med prescription and administration are the third leading cause of death in the United States. There are tools available such as the Beers Criteria that assist the treating clinician to identify medications that are potentially inappropriate for the older adult. The purpose of this course is to equip clinicians with medication knowledge and tools to assess for intended and adverse effects of medication. The participant will be able to perform a medication reconciliation to ensure drug safety regarding proper drug dosing (dose and timing), indications, contraindications, drug-drug interactions, and how physical and mental functions may be impacted by the medications our patients are taking.

Upon completion of this course participants will be able to

  1. Complete a medication reconciliation for older adults experiencing polypharmacy as part of the drug regimen review process.
  2. Identify and communicate adverse drug reactions and side effects to health care team in order to improve patient safety.
  3. Incorporate the 2019 American Geriatric Society Potentially Inappropriate Medication Resource (aka “Beers List” into clinical practice.

  • 0-20 min – Introduction/Background/Definitions
  • 25–55 min – Drug errors, commonly prescribed medications, most common drugs related to rehospitalization
  • 55–85 min – Beers Criteria and Potentially Inappropriate Medications, deprescribing
  • 85–100 min – Regulations, medication safety, role of therapists, medication effects on function
  • 100–113 min – Steps to Med Reconciliation
  • 113–126 min – Video demo of history taking and medication review
  • 126–140 min – Practice Case, Assessment tools
  • 140–153 min – Patient education, Over the counter (OTC) medication labeling, side effects, tolerance/withdrawal, and addiction
  • 153–173 min – General and local anesthesia, medications and physical function, incontinence
  • 173–180 min – Video case of med reconciliation, conclusion and questions/answers

Upon completion of the course, a certificate of completion for 3 contact hours of educational credit will be awarded.
